Community Health Alliance is an employer that offers career opportunities with purpose.Community Health Alliance is looking for a nurse to join our team at our Medical offices in Hamilton and Middletown, Ohio.At Community Health Alliance our employees are our greatest asset so we offer a full benefit program designed to support you and your family.We are proud to offer a comprehensive benefits package:Affordable medical, dental and vision insuranceCompany paid Health Reimbursement Account (HRA)Generous paid time off (PTO) & paid holidays403b, with company match - up to 6%Public Service Loan Forgiveness (PSLF) eligible employerCareer ladder - Start here and build a lifelong careerCompany paid long-term disability and life insurancePaid license renewalJob Duties and Responsibilities:Complete nursing tasks as directed by a registered nurse or medical provider.Administers medications to patients when appropriate and maintains accurate records.Participates as part of an interdisciplinary team coordinating patient care.Monitors patient appointments and collects health information and vitals for medical provider.Educates patients on monitoring and managing symptoms, medication and health conditions.Collects blood, urine and other specimens as needed for laboratory testing.Administers injections, TB testing, vaccinations as ordered by medical provider.Completes health and symptomology screenings to aid in patient care.Assist in scheduling patient appointments as needed.Coordinates with other medical facilities to obtain medical records for treatment of patients.Implements appropriate nursing interventions and actions according to established plan of care within scope of practice.Administer basic first aid, when necessary.Accurately and timely documents patient information and interventions in the electronic health record.Qualifications:Licensed in the State of Ohio as a Licensed Practical Nurse (LPN).Two years related experience.Current BLS/CPR or willing to obtain within 30 days of hire.Ability to collaborate with all members of the healthcare team.Effective verbal and written communication skills.Exceptional customer service skills.Must be able to work as a team member.Proficiency in Microsoft Office products and documentation.Community Health Alliance is a leader providing the highest industry standards for health and human services.
