Job Description:
Job Summary The Coordinator Diabetes Education Is responsible
for development and coordination of inpatient and outpatient
diabetes education that includes both the patient and
family/significant others whenever possible. The coordinator serves
as a resource for hospital staff, medical staff, and the
community. Job Responsibilities Provide administrative and
coordination oversight for both inpatient and outpatient Diabetes
Education programs. Includes the tracking and maintenance of
diabetic patient data and the planning and implementation of
individual and group classes. Work with appropriate organizational
resources to promote diabetes education classes to hospital staff,
medical staff, and the community. Formulate, review, document, and
develop educational materials and patient record forms as needed.
Coordinate the education of diabetic patients and significant
others, collaborating with nursing staff in the daily
identification of patients who need instruction and follow-up
education. Assess the learning needs of each patient and establish
an individualized teaching plan with stated educational objectives
for each patient. Attend unit patient care conferences as needed
and serve as a resource person for the patient and authorized care
givers following discharge. Education may include but is not
limited to: Instruction regarding SBGM (self-blood glucose
monitoring), medication, exercise and general diabetes care to
patients, families, and nursing staff. Provide written and verbal
post-hospitalization discharge instructions regarding diabetic
treatment regimen at home. Consult with providers regarding patient
nursing and educational needs and partner with Social Service for
coordination of patient home care needs. Provide both written and
verbal communication to providers regarding referral process for
education classes. Participate in the revision and review of
diabetes related nursing policy and procedures and submit to
Nursing Policy and Procedure Committee for approval. Serve as a
liaison with pharmaceutical and supply representatives to
facilitate exchange of information of current products and
literature available. Other duties as assigned. Education
Requirements Minimum: Health Care Professional who is a Certified
Diabetes Educator Experience Requirements Minimum: Three (3) years'
experience in clinical nursing / dietitian practice. Must have
acquired specialized knowledge related to the care of the diabetic
patient. License/Certification Requirements Registered Nurse,
Registered Dietitian, Pharmacist or Physician who is a CDE or has
achieved Board Certification in Advanced Diabetes Management
(BCADM)
