Must-Have: Experience managing a caseload of around 25 patients.
Acute inpatient case management experience. Description:
Responsible for facilitating the progression of care. Evaluate for
the next appropriate level of care. Assist with discharge planning.
Work in an adult inpatient unit that may include med/surg,
