Primary Care Physician Columbus, OH COMPENSATION: Salary $250k
Incentive Bonus Signing Bonus up to $50k We are a physician-led and

and clinical leaders that are seeking to fulfill purpose and
personal opportunity and join the team. Transportation is provided
to patients that need it at no charge. Role: Compensation: Base
$250k Bonus: Up to 10% of Base Salary Partnership: No Buy-in.

patients a day Hours: Monday Friday work schedule; Clinic Hours
7:30a 5:00p (last patient is scheduled at 4:15) Work/Life Balance
Small Patient Panel 450 patients max Administrative time included
in schedule No nights or weekends 100% outpatient Dedicated Care
and support team (have your own MA/Scheduler/Referral specialist)
Patient Population: Seniors with 3-5 chronic diseases Value Based
