Care Coordinator II: - Completion of a Medical Assisting program
required - LPN license required or Certified/Registered MA required
- Active BLS required - EMR experience preferred Care Coordinator
II are an essential part of our team and make a real difference in
people’s lives. They fulfill patient care duties and administrative
tasks, liaising between patients and physicians. - Provides
telephonic outreach to patients identified as needing follow-up
from ordered referrals to specialty care and diagnostic testing.
Patients with orders may have chronic health conditions and need
preventative services. - Addresses and coordinates Transitions into
Care after a Hospital discharge. - Assist in identifying gaps in
care utilizing NextGen and Eagle Dream/Population Analytics. Care
Gaps include cervical cancer screenings, colon cancer screenings,
breast cancer screenings, well child checks, immunizations, and
