POSITION: Community Health Worker

GENERAL DESCRIPTION: Someone who shows passion for family advocacy, a unique ability to empower families, and an ability to apply their extensive community health knowledge toward desired resources. Able to accept referrals from information and referral sources, parents, physicians, and agencies for services to children 0-3 years, conducts initial interview and home visit and collects related reports or records. Performs required screenings/ assessments/evaluations. Collaborates with other providers to facilitate a "seamless" service system. This staff member will communicate with members of the community to determine needs and help to link African American moms and babies to essential services, but not to the exclusion of any family in need of service. The CHW will provide ongoing home visits and act as an advocate for the underserved population.

* A minimum of two (2) years' experience with young children and their families preferred, familiarity with agencies serving at-risk populations and a demonstrated ability to work cooperatively with local agencies.
* Community Health Worker Certification preferred.
* Knowledge of agencies and community resources.
* Experience in engaging parents, community providers and community leaders
_(occasional evening and weekend hours may be needed)_.

* Willingness/ability to work as a team member.
* Good organizational skills.
* History of good job attendance.
* Valid Ohio driver's license with good driving record with daily access to private insured vehicle during work hours.
* Ability to work flexible schedule to meet the needs of the program.
* Excellent communication skills: verbal and written.
* Must pass criminal background checks and be free from communicable disease.

* Communicates with members of the community to determine needs and help link African
American moms and babies to essential services.

* Facilitates family involvement in referral to other agencies as needed.
* Encourages and fosters the family's independence in decision making about the services they are receiving.
* Collects and maintains files on all children referred and/or served. Collect and maintain files on community outreach.
* Participates in Program evaluation activities as requested by the Program Director.
* Plans and organizes own work to meet priorities.
* Collect, input, analyze, maintain and report on program requirements within data base.
* Maintains confidentiality.
* Continuously updates and improves professional competence.
* Complies with state and federal regulations that pertain to the position _(e.g. OSHA: _
_Occupational Safety Hazard Administration, ORC; Ohio Revised Code)_.

* Performs other duties consistent with the position as assigned.
* Provides services in a culturally competent and respectful manner.
* Provides ongoing visits in the home.
* Promotes positive parent child interaction, bonding, attachment and school readiness.
* Meets regularly with supervisor to report data outcomes.
* Required attendance at any staff meetings, professional development and infant mortality trainings or conferences.
* Will attend curriculum training to be able to provide education to parents.

* To serve as a role model for students in how to conduct themselves as citizens and as responsible, intelligent human beings.
* To help instill in students the belief in and practice of ethical principles and democratic values.

* Knowledge of: Board policies and procedures, building policies and procedures, public relations and state reporting.
* Ability to: Interpret policies and procedures, follow instructions, communicate effectively, and maintain files and reports, collect data, schedule, and lift moderate amounts of weight. To provide own transportation to do home visit.

Copier, fax, computer, and general office equipment.


Occasional: Exposure to unruly children/adults, blood, bodily fluids and tissue. Exposure to smoke, animals, and contagious illness/conditions while visiting homes.

Salary and work year as established by the Governing Board. The employee shall remain free of any alcohol or non-prescribed controlled substance use in the workplace through his/her employment in the district.


Performance of this position will be evaluated in accordance with provisions of the Board's policy on Evaluation of Support Personnel.

STARTING DATE: As soon as possible
SALARY: $16.00 - 18.00 based upon education and experience
BENEFITS: Excellent benefits for all full time employees
WORK YEAR: Full time - 260 days
